In order to be Eligible to participate in District 112 Activities or Athletics:
1) Athletic Eligibility Form and/or Fine Arts Eligibility Form (for activites) needs to be filled out each year and signed by a parent/guardian and the athlete on the last 2 pages.
2) MSHSL Sports Qualifying Physical Examination Clearance Form needs to be completed every 2 years, filled out by a family physician, and
    signed by a parent/guardian and the athlete.  This form MUST be on file in the CHS Activities Office before a student can participate.
3) Chaska High School Activities Card needs to be completed each year and signed by a parent/guardian.  This 4 part form can be found at
    the Chaska High School Activities Office.
4) The completed forms and the fee must be turned into the CHS Activities office one week before the start date of the sport or activity.
    These forms must be delivered to the Activities Office.  Any equipment from previous athletics or activities that the student has
    participated in needs to be returned to the prospective coach/advisor before they can compete.
